FOR LANDLORDS/PROPERTY OWNERS ONLY
I authorize Burlington Electric Department (BED) to either disconnect or continue electrical service automatically when my tenant calls to cancel their service in accordance to what I have checked off below. If I have chosen option 1 or 2, I will assume responsibility for all bills rendered after the tenants requested cancellation date unless I notify BED to disconnect service. I understand that this request means that BED does not manage my properties and will not notify me prior to carrying out these instructions.
 
Please select one of these three options below.

1. Leave the power on ANYTIME, WINTER OR SUMMER, when a tenant contacts BED to cancel their electrical service. I understand the power will be transferred in to my name and a $30.00 service charge will appear on my first bill. I understand that if my tenant should have their service disconnected for non-payment, the power WILL BE shut off and not transferred automatically in to my name.
2. Leave the power on ONLY IN THE WINTER MONTHS, from NOVEMBER 1st to MARCH 31st when a tenant contacts BED to cancel their electrical service. I understand the power will be transferred into my name only during those months and a $30.00 service charge will appear on my first bill. I understand that from April 1st to October 31st the power will be shut off if a tenant contacts BED to cancel their electrical service. I understand that if my tenant should have their service disconnected for non-payment, the power WILL BE shut off and not transferred automatically in to my name.
3. Disconnect the power ANYTIME, WINTER OR SUMMER, when a tenant contacts BED to cancel their electrical service. I understand that BED will not be liable for any damage which may result in such disconnection of power.
